Immunization and Multiple Contacts in the Form Builder Now Live

NEW: Immunization Information Collection

What's New

Parents can now submit and update their child's immunization records directly through Parent App, based on the immunization schedule you've configured in your settings.

How It Works

  • For Centers: Set up your required immunization schedule once in your admin settings
  • For Parents: They'll see a clear, easy-to-follow form showing which immunizations are needed based on their child's age

Why This Matters

No more filing cabinets full of vaccination cards. No more manually tracking who needs what shot when. Just clear, organized, digital health records that keep everyone safe and compliant.

Getting Started

  1. Navigate to Settings → Children → Immunization Schedule
  2. Configure your required immunizations based on your local regulations
  3. Add Immunization to the Form Builder
  4. Parents will automatically receive prompts to upload documentation

NEW: Mandatory Multiple Emergency Contacts

What's New

You can now require parents to provide multiple emergency contacts before they can complete enrollment or access certain features.

How to Enable

  1. Go to Smart Tools → Build New Form → Add Contact
  2. Go to Setting within the Form Builder → Toggle ON "Allow Parent to Add Multiple Contact"
  3. Set your minimum number of required contacts
  4. Make the field mandatory

What Parents Experience

  • Clear prompt explaining why multiple contacts are essential for child safety
  • Easy-to-use form for adding contact details
  • Ability to set contact priority and specify relationships
